Diabetes Support Groups

 

Online Self Management Course for Long Term Conditions 

This is available to Angus residents living with any long term health conditions to help them adjust to living well with long term conditions.  To book a place please contact Rhona Guild or Janice Reid on 01307 474889 or

Angus Exercise Referral Programme is available to people with a long term health condition including Type 2 diabetes or glucose intolerance; older adults; and / or those new to exercise who don't know where to start. It provides people with the opportunity to take part in an exercise programme which exactly meets their needs.  It takes place at local sports centre in Brechin, Montrose, Forfar, Kirriemuir, Arbroath and Carnoustie.   Forms for referral are available from Angus sports centres, Angus GP Practices or any Angus health professional.

 

Angus Cardiac Group is a partner in the Be Active Live Well scheme and also offer support to all people living in Angus who suffer from heart disease.  Further information is available by clicking on the website below

Angus Long Term Conditions Support Groups support for anyone with a condition requiring ongoing medical care.  There are four self help support groups who meet regularly in Edzell, Montrose, Carnoustie and Forfar.  Further information is available from the contact details below:

Angus Diabetes Forums are free information meetings open to Angus residents living with Type 2 diabetes and are held in Arbroath, Forfar and Montrose.  Information on dates is available through Angus general practices or on www.diabetes-healthnet.ac.uk
